The allergies box is the one that matters most. If there is nothing, write “none” rather than leaving it blank, so we know it was answered rather than skipped.

Photographs

I am happy for photographs of my child to be used in the school’s newsletter and on its website and Facebook page.

This is genuinely optional and you can change your mind at any time — tell us and we will take anything down. Leaving it unticked does not affect your child’s place, and no photograph of them will be published.

Nothing is charged here. We confirm the place and the fees first.

What happens to this

It goes onto your child’s record, which only the school can see. Medical details are held because the school needs them to look after your child safely, and they are not shared with anyone else.
